Privacy, Patient Information & Cookies

This page explains how the public website handles information and how clinical information, privacy requests, and patient records are managed separately.

Clinical care and Jane

The public contact form and ordinary email are for brief, non-clinical service-fit questions only. They are not used for clinical intake, care records, or payment-card information.

Booking, clinical intake and consent forms, secure care messages, clinical and financial records, invoices, receipts, and payments are handled through Jane and its authorized service providers. Payments are processed through Jane's integrated payment provider. The practice remains responsible for the information.

Health information practices

Tysen Pritchard, R.Kin, B.Kin, operating as Eventus Athletics, is the Health Information Custodian for this practice. The practice collects, uses, and discloses only the information reasonably necessary to provide and document care, communicate with you, bill for services, and meet legal and professional obligations.

Information is disclosed only with consent or when permitted or required by law. Records are protected and retained in accordance with the Personal Health Information Protection Act and College of Kinesiologists of Ontario requirements.

Access, correction, and privacy complaints

To request access to or correction of your record, ask a privacy question, or make a privacy complaint, contact Tysen through Jane if you are an existing client, or email kin@eventusathletics.ca. Do not include health information in ordinary email. You may also make a complaint to the Information and Privacy Commissioner of Ontario at ipc.on.ca or 1-800-387-0073.

How this site uses cookies and local storage

Essential cookies keep the site working. If you opt in, preference storage can help you continue a pathway, restore part of the inquiry flow, or remember selected tool settings and presets on the same device. Analytics, when enabled, are used to improve the site experience and understand site usage at a basic first party level.

Contact inquiries and email

The contact form collects your name, email address, a broad service-fit topic, an optional city and province, and a short question. These details are used to review and respond to your inquiry.

Form submissions and email replies pass through third-party email delivery, routing, and inbox providers so they can reach the practice inbox. This is ordinary email, not a clinical record in Jane.

Do not use the contact form or ordinary email to send medical or health information, medical history, diagnoses, treatment details, or documents. The form does not accept file uploads. Existing clients and anyone with a clinical matter should use Jane.

Additional privacy notes

  • Saved progress, tool settings, presets, and return visitor guidance stay on this device and are not used for cross site ad tracking.
  • The site does not use condition specific retargeting or invasive cross site tracking related to health related browsing.
  • Inquiry drafts are stored locally in a conservative way and do not save the message field.
  • You can reopen cookie preferences at any time and switch optional categories off again.
