This page explains what information the website uses, where health information belongs, and how to ask a privacy question.
The short version
- Use Jane for booking, payment, health information, records, and care messages.
- Use the public form or email only for short, general questions.
- Optional preferences and analytics stay off until you choose them.
- You can ask to see or correct your record and can make a privacy complaint.
Clinical care and Jane
Use the public form and ordinary email only for short questions about the service, booking, or payment. Do not use them for health intake, care messages, documents, or card information.
Even a general question about a health service can reveal personal health information. Eventus will handle any information it receives under its privacy duties.
Jane handles booking, health intake and consent forms, secure care messages, records, invoices, receipts, and payments. Jane uses approved service providers, including its payment provider. Eventus remains responsible for the information.
Health information practices
Tysen Pritchard, R.Kin, B.Kin, operating as Eventus Athletics, is the Health Information Custodian. This means he is responsible for the personal health information held by the practice. Eventus collects, uses, and shares only the information needed to provide and record care, communicate with you, bill for services, and meet legal and professional duties.
Information is shared only with consent or when the law allows or requires it. Records are protected and kept under the Personal Health Information Protection Act, also called PHIPA, and College of Kinesiologists of Ontario requirements.
Access, correction, and privacy complaints
To request access to or correction of your record, ask a privacy question, or make a privacy complaint, contact Tysen through Jane if you are an existing client, or email kin@eventusathletics.ca. Do not include health information in ordinary email. You may also make a complaint to the Information and Privacy Commissioner of Ontario at ipc.on.ca or 1-800-387-0073.

Contact inquiries and email
The contact form collects your name, email address, a broad service topic, an optional city and province, and a short question. These details are used to review and respond to your inquiry.
Form submissions and email replies pass through external email delivery, routing, and inbox providers so they can reach the practice inbox. These channels are separate from Jane and should not be used for clinical communication.
Do not use the contact form or ordinary email to send symptoms, medical history, diagnoses, treatment information, photographs, documents, or payment details. The form does not accept file uploads. Existing clients and anyone with a clinical matter should use Jane.
Cloudflare Turnstile protects the inquiry form from automated abuse. When the form is used, Cloudflare processes the IP address and basic details about the browser, connection, and page. Cloudflare states that Turnstile does not access, store, or send the information typed into the form. This security check is required to use the form. You can email Eventus with a general question instead. Use Jane for health information or care questions. Learn more in Cloudflare's Turnstile Privacy Addendum.
Temporary security counters use coded versions of the IP address and email address. They do not contain the inquiry message and expire within six hours.
Additional privacy notes
- Saved progress, tool settings, and return prompts stay on this device. They are not used for tracking across other sites.
- The site does not target ads based on health related pages you view.
- Saved inquiry progress does not include the message field.
- You can reopen cookie preferences and turn optional choices off at any time.
